Remedy Soul Care, PLLC accepts Aetna, Cigna, UHC/Optum, most Blue Cross Blue Shield, and a few other insurance policies at this time. Please check with your insurance policy to verify that you are covered for behavioral or mental health services and that I, Kam Duncan, am listed as an in-network provider. Also, please note some important considerations for billing insurance:

  • Insurance or managed care companies (MCO’s) determine what they will and will not cover and how many sessions are allowed within a year.
  • Insurance companies require a diagnostic code, which becomes part of the individual’s medical records. Some clients seek therapy for reasons that do not warrant an insurance-approved diagnostic code and/or prefer to not have a diagnostic code as part of their medical history.
  • Prior to your appointment, please verify your individual policy plan regarding out of pocket cost in which you are responsible. This includes deductibles, copays, or co-insurance costs.

Client Forms: New client forms can be emailed to those who prefer to complete them prior to the first session or you may arrive 20 minutes early to complete the forms in the office so valuable session time is not taken up by this task. Please note that email is not always a secure (confidential) method of transmission.

A Release of Information form can be completed for those who may want or need to request a release of information to or for doctors, family, a responsible party, or legal professionals. This form allows you to decide how much or little confidential information is shared.
